Community Health Worker Qualification and Education Requirements - High School Diploma or equivalent
- Completed TDH approved CHW Certification course
- Current Driver's License & Reliable Transportation
- Excellent observation, communication and relationship building skills
- Proven experience in healthcare or community outreach role
- Ability to work independently and collaboratively within a team
- Experience with EMR software is a plus
Essential Duties - Assists with recruitment and enrolment or participants in home setting
- Provides emotional support to patient and their families, fostering open communication and support
- Assists staff in identification and connection to resources for patients and families
- Develops relationships with community organizations and facilities to secure resources and helps to complete appropriate applications
- Assists in coordinating home health services, medical equipment and things related to medication management
- Maintains accurate and up to date documentation of up-to-date documentation of patient interactions and interventions
- Ensure compliance with all relevant healthcare regulations and standard
- Performs other duties as required.
